Shoulder joint replacement is technically a very complex and complicated procedure that may fail in some cases to produce a satisfactory restoration of comfort and function due to various reasons.

Causes of failure of shoulder joint replacement

  • Glenoid arthritis caused by inadequate centering, sizing and stiffness or malposition
  • Periprosthetic humeral fracture
  • Glenoid loosening
  • Other component loosening
  • Prosthetic mal-positioning
  • Osteolysis
  • Capsular contracture
  • Blocking osteophytes
  • Overstuffing (too large prosthetic components)
  • Proximal humerus fracture complications
  • Joint instability
  • Deficient or non-functional rotator cuff
  • Non-anatomic prosthesis
  • Subscapularis detachment
  • Nerve injury
  • Deltoid detachment
  • Disuse atrophy of the muscles
  • Soft tissue imbalance
  • Soft tissue ingrowth
  • Inadequate rehabilitation

Probable symptoms of failed total shoulder replacement surgery

The shoulder replacement surgery is performed with an aim to reduce the pain and improve the functioning of the joint by replacing damaged parts of the joint with artificial prosthesis. But in some cases, shoulder joint does not show signs of improvement and surgery may be considered a failure. The main symptoms of failed total shoulder replacement surgery include the following:

  • Persistent pain
  • Increases the stiffness of the shoulder joint that doesn’t allow the normal range of motion even with some help
  • Inability to perform regular activities
  • Shoulder joint infection
  • Frequent dislocations or subluxations
  • Joint weakness
  • Crepitation, clicking, or clunking sound from joint
Revision procedures

In cases of complications or failure of the shoulder replacement surgery, revision surgery is generally required.

Revision depends upon the type of failure and may include any of the following:

  • Revision of the component
  • Bone graft
  • Impaction grafting
  • Soft-tissue repair
  • Implant removal
  • A primary or secondary exchange procedure
  • Reverse shoulder replacement
  • Cement removal
